In the Matter of the Application of LAKESIDE HEALTH SYSTEM, INC. For an Order Approving Dissolution Pursuant to Section 1102(a)(2)(E) of the Not-for-Profit Corporation Law.
Upon the Verified Petition of Patricia Hayles, Chair of Lakeside Health System, Inc., dated Aug. 15, 2025, it is hereby:
ORDERED that the above-named Corporation, and all persons interested in the above-named Corporation, show cause before this Court at the Monroe County Supreme Court, 99 Exchange Blvd., Rochester, NY 14614, on the 14th day of January, 2026, at 9:00 AM, or as soon thereafter as the parties can be heard, why an order should not be made and entered pursuant to NY NPCL § 1102(a)(2)(E) approving the dissolution of the above-named Corporation and the filing of a Certificate of Dissolution effecting such dissolution, and granting such other and further relief as the Court deems just and proper; and it is further
ORDERED that service of this Order together with a copy of the papers upon which it is granted via first class mail upon:

ORDERED that a copy of this Order be published in The Daily Record at least once in each week from December 24, 2025 to January 14, 2026, inclusive, those being the three weeks preceding the first-mentioned hearing; and it is further
ORDERED that within 10 days of entry of this Order that a copy of this Order be filled with the Monroe County Clerk as required by NY NPCL § 1104(d)
